Acceptance of our Terms and Conditions establishes the professional agreement between you and Down the Vets and enables us to provide a safe, efficient and high-quality veterinary service for both you and your pets.

Submitting this form does not automatically mean that you have become a client of Down the Vets.
A veterinary-client relationship is only established after your application has been reviewed and you have received written confirmation from Down the Vets that your registration has been accepted.

By submitting a registration form and subsequently accepting veterinary services from Down the Vets, you agree to the following Terms and Conditions.
Submitting a registration form does not automatically make you a client of Down the Vets.
All applications are considered individually and are subject to acceptance by the practice.
You will only become a registered client once you have received confirmation from Down the Vets that your registration has been accepted.
Until that time:
You are not considered a client of Down the Vets.
You must continue to make arrangements for veterinary care through your existing veterinary practice.
You must not state or imply that you are a client of Down the Vets.
Down the Vets reserves the right to accept or decline any application for registration without being obliged to provide a reason.
Down the Vets is an independent mobile veterinary practice.
Consultations are provided by home visit and by prior appointment only.
The practice does not provide consulting rooms, hospitalisation, inpatient facilities or routine surgical facilities.
Many medical conditions can be investigated and treated during a home visit. However, some illnesses or injuries require more advanced diagnostic equipment or hospital care.
Where, in the opinion of the veterinary surgeon, further investigations or treatment are required, you agree that your pet may be referred to Bollington Vets for services such as:
X-rays
Ultrasound examinations
CT or MRI imaging
Blood tests requiring hospital facilities
Anaesthesia
Surgery
Hospitalisation
Intensive nursing care
Other specialist investigations or treatments
You understand that these services are provided independently of Down the Vets and that you will be responsible for all fees charged by Bollington Vets.
Down the Vets does not provide continuous emergency cover outside normal consulting hours.
If emergency veterinary attention is required outside normal opening hours, clients will normally be directed to Vets Now, who provide dedicated out-of-hours emergency veterinary services.
You understand and agree that:
All fees charged by Vets Now are payable directly to Vets Now.
Those fees are entirely separate from the fees charged by Down the Vets.
Down the Vets has no control over the fees charged by Vets Now.
Where your case is referred to Vets Now by Down the Vets, an administration and referral fee of £75 plus VAT will be payable to Down the Vets.
Payment is due at the time veterinary services are provided.
Payment may be made using any payment methods accepted by the practice.
Accounts are not normally offered.
Any additional items including, but not limited to:
Medicines
Injections
Prescription medications
Laboratory tests
Diagnostic tests
Microchipping
Certificates
Prescriptions
Referral administration
will be charged in addition to the consultation fee where applicable.
Where your pet is insured, payment remains your responsibility.
Unless otherwise agreed in writing, clients are expected to pay the account in full at the time of treatment and reclaim the costs from their insurer.
The veterinary surgeon will always act in the best interests of your animal's welfare.
Clinical recommendations, treatment plans and referrals are made solely on professional veterinary judgement.
Owners remain free to accept or decline recommendations, although the veterinary surgeon reserves the right to decline continued responsibility where recommended investigations or treatments are refused and this may compromise animal welfare.
Down the Vets aims to provide a friendly, respectful and professional service.
The practice reserves the right to refuse registration or discontinue providing veterinary services where abusive, threatening or unreasonable behaviour occurs.
Personal information and clinical records will be held securely and processed in accordance with UK data protection legislation.
Information will only be shared where necessary for the care of your animal, including with referral practices, laboratories and regulatory authorities where appropriate.
By requesting registration with Down the Vets, you confirm that:
You have read and understood these Terms and Conditions.
You understand that submitting a registration form does not make you a client.
You understand that you will only become a client after your application has been formally accepted.
You understand that Down the Vets is a mobile home-visit veterinary practice.
You agree that payment is due at the time of treatment.
You agree that referral to Bollington Vets may be necessary for investigations, surgery or hospitalisation.
You agree that emergency out-of-hours care will normally be provided by Vets Now.
You accept responsibility for all fees charged by Vets Now.
You accept responsibility for the Down the Vets referral administration fee of £75 plus VAT where applicable.
You agree to comply with these Terms and Conditions.
These Terms and Conditions may be amended from time to time. The latest version will always be available from Down the Vets upon request.
